Backflow Device

For this project, you will be required submit:

• Application to construct or demolish 
• Schedule 1 designer Information - located on page 3 of the permit application form
• Owner's authorization form
• Copy of Durham Region's cross connection survey (completed by a plumber on the Durham Region's list of qualified testers) 
• Copy of the manufacturer's form for sizing the expansion tank
• Copy of Durham Region's notice letter to install the device(s)
• Name of plumber and Ontario College of Trades membership number
• Size and type of device

Please note: Engineered drawings and commitment to general review is required when backflow services are:

• 2-1/2 inches and greater, or
• installed on fire systems, regardless of size
